This is a screen capture of the DICOM Image Archive Import DICOM Images page.

The User Input area shows a list of five numbered input fields (in the format DICOM Image File n:), which are blank and followed by a Browse... button.

At the bottom of the page, these buttons are shown: Cancel, Reset, and Import.
