This is a screen capture of the DICOM Image Archive Administration Add Repository Documents page, with the inserting document category shown as CONSTRAINT and with the following message "1 CONSTRAINT Documents have been added to the repository."

The Browse to select the document files: region includes a list of three numbered input fields (in the format Document file n:), which are blank and followed by a Browse... button.

At the bottom of the page, these buttons are shown: Back and Add.
